Advance care planning and My Health Record – find out more and share feedback

Advance care planning can help you to plan ahead, so your family, friends and doctors will know what you want if you can no longer speak for yourself. My Health Record is an online summary of your key health information.

You can store advance care planning documents on My Health Record so that your healthcare team all have access to the information about your plans.

Health Consumers’ Council, Carers WA, the Advance Care Planning Consortium (via Palliative Care WA) and WA Health Support Services (part of WA Health) are organising a workshop in February 2020. You can register your interest via the email address below.

Diabetes Lived Experience Working Group members sought – closing date 31 December 2019

Call for expression of interest

The Diabetes and Endocrine Health Network (DEHN) at the WA Department of Health is calling for expressions of interest for members to join its Lived Experience Working Group (LEWG).

The team are looking for people who are passionate about continuous improvement and improving the lives of people with diabetes and those who care for them.

The purpose of the Lived Experience Working Group is to represent the collective voice of consumers and carers while balancing the diversity of experiences across different groups such as diabetes types, regional and remote areas, etc. and engage with a wide range of people living with diabetes and endocrine health issues with diverse experiences and backgrounds.

Consumer participation payments are available – see the information in the draft terms of reference for more details.

To apply please complete the application form by Tuesday 31 December 2019.

Please read the Expression of Interest attached or contact Health Networks at healthpolicy@health.wa.gov.au for more information. The Terms of Reference will be confirmed once the group has been established. It’s expected the first meeting will be in early February.
